Find In-Home Care Services

Expand the types of settings in which individuals can receive comprehensive long term care under Medicaid which offers an alternative to institutionalized care.

Support services include supported living services for adults 18 years and older and the children's extensive support program for children through age 17.

Residential Habilitation Personal Care Alternative Or Host Homes

Individual Residential Services and Supports (IRSS) use a variety of living arrangements individually designed to meet the unique needs for support, guidance and habilitation of each person receiving services. The program approved service agency has the responsibility for the living environment and persons may live in a home owned or leased by the agency, their own home or a Host Home. Services are generally provided to no more than two persons receiving services per setting. Living environments utilized for IRSS shall be selected in accordance with Department guidelines implementing section 1616(e) of the Social Security Act (42 U.S.C. section 1382e (e)(l)) and addressing building capacity, including the maximum number of persons receiving services and the number of units which may be utilized under a single roof or in a single setting.

Day Habilitation/supported Community Connections

Day Habilitation Services and Supports provide training, support and supervision activities which maximize functional abilities and skills necessary to enable adults to access the community and/or provide the basis for building skills which will assist individuals to access the community. Day Habilitation Services and Supports are to be provided outside of the person's living environment, unless otherwise indicated by the person's needs, through meaningful employment, activities and community participation. If services cannot be provided outside of the living environment due to a person's medical or safety needs, this shall be documented. Integrated activities services which utilize the community as learning environment to provide individuals access to, and participation in, typical activities and functions of community life. These services provide a variety of opportunities to facilitate relationships and natural supports in the community through activities such as volunteer work, community education or training and retirement activities. Non-integrated work services which are focused on providing supervision to persons who are engaged in remunerative work and instructions, as needed, to perform remunerative work. These services are provided in sheltered/segregated settings in which the majority of individuals have a disability or the primary purpose of the agency/business is to provide training or day activities for individuals with disabilities. 

Non-Medical Transportation

AIM provides transportation to persons receiving services in vehicles owned and operated by the agency has implement written policies and procedures in accordance with department policies and procedures and which shall include, but not be limited to:

  1.  Insurance coverage
  2.  Safety equipment
  3.  Vehicle condition and maintenance
  4.   Emergency procedures
  5.   Personnel qualifications
  6.   Use of aide
  7.   Volunteers
  8.   Record keeping

Children's Extensive Support Services

Children's Extensive Support Services are intended to provide the services and supports to children most in need because of the severity of the disability and provide for stability of the family setting which would allow the child to continue to remain in the family home.

Children's Extensive Support Services are intended to provide the services and supports to children most in need because of the severity of the disability and provide for stability of the family setting which would allow the child to continue to remain in the family home. AIM does not replace typical parental responsibility, but enhances and supports the care provided by parents. The specific array of services and supports for each person will vary depending on the individual's choices and needs.

The child participating in CES shall live at home with his/her biological, adoptive parent(s) or guardian, or be in an out of home placement and being returned home with the provision of the program.

There shall be a record that the child's physician has certified that the medical services and supports identified in the Individualized Plan are sufficient to meet the child's needs in the home setting.

Supported Living Services

Medicaid funded supported living services for adults are provided through the home and community based services program which is described in the Colorado Department of Health Care Policy and Financing rules and regulations, Medical Assistance Staff Manual, section 8.500(10 C.C.R. 2505-10) and the Department's program descriptions. State funded supported living services for adults are provided pursuant to the Department's program description.

Basic & Enhanced Homemaker Services

  1. Basic homemaker services include regular household cleaning and maintenance only in the areas where a client frequents most.
  2. Enhanced homemaker services include all tasks assigned with basic services with additional focus on high traffic areas where germs and viruses can live for an extended amount of time, such as door knobs, light switches, stair railings, etc.& toddler zones enhanced services are those tasks that are beyond routine sweeping, mopping, dusting or cleaning and requires deep disinfecting and sanitizing in areas which most often go overlooked. Our enhanced services ensure a safer and healthier living environment for the individual with a disability and the family.

    Both household services are conducted in prevised areas of the immediate home only. Services include but are not limited to:

    • Living, kitchen, bedroom & bathroom areas
    • Indoor windows, walls, baseboards, stairways & upholstery 
    • Debris & trash removal, dusting, sweeping, mopping, dishwashing and polishing of wood & stainless-steel fixtures
    • Bedroom linen changing, laundry, structure and organization as needed
    • Deep disinfecting and sanitizing of light switches, doorknobs, electronic items, countertops, chairs, stair rails & bathroom fixtures.

    Habilitation services shall include direct training and instruction to the client in performing basic household tasks including cleaning, laundry and household care which may include hands-on assistance by performing a task on their own or enhanced hands-on training, prompting and cueing as necessary, with the main intent being to increase independence of the client. 

    Our providers will be engaged and present tp provide step-by-step verbal or physical instructions throughout the entire task at hand.

    Due to the increasing concerns surrounding the novel coronavirus pandemic, aim has taken a continued and ongoing approach to minimize and effectively use preventative action to decrease and eliminate the spread of the virus. all staff members are national caregiver COVID certified & trained in universal precautions. each residence that is provided services will require all staff to wear full personal protection equipment (PPE) upon arrival at the start of services.
